Training Advanced Accident Investigation and Management
Although international trends show a decline in the overall total number of accidental deaths and work place accidents, the associated financial costs, both direct and indirect, continue to rise. Many oil and gas companies continue to have employee accidents despite modern industry having the knowledge or know-how to prevent such accidents. There is an obvious requirement that those professionals undertaking enforcement or regulatory activities in a team must have competence to undertake efficient and analytical accident/incident investigations, which may in the course of due legal process be further examined and scrutinized by legal tests. This course has been designed to meet the requirements of Principle and Lead Regulatory and enforcement officers who are required to undertake competent accident investigations and examinations across a wide range of industries, including the hydrocarbon industry. The program includes practical exercises as well as a detailed review of accident events in oil and gas companies resulting in a fatality involving a number of hazard and risk profiles. A number of Root Cause Analysis (RCA) tools will be reviewed and applied during the course.
OBJECTIVES
- Understand the roles and responsibilities of all those involved in accident/incident reporting and investigation and the methods used to monitor the implementation recommendations resulting from these investigations
- Explain the causes of accidents and review and apply various accident causation models
- Understand and appreciate the objectives of the various methodologies for undertaking the Root Cause Analysis (RCA)
- Apply the role of RCA in the development of corporate occupational and environmental risk management systems
- Apply the tools TOR and MORT to accident investigations including in a pro-active audit role
